Appointment required. Merchant contact information will be available for you after purchase in your voucher. Reschedules must be made at least 12 hours before your appointment. Anything less will require a new deposit. 15 minute grace period; after that, appointment will be forfeited. A non-refundable $15 service fee applies, payable directly to the merchant. Groupon does not collect or process this fee. Arrive with clean lashes (no makeup, mascara, or caffeine beforehand). Must be at least 17 years old and sign a consent form. For fills, at least 40% of lashes must remain after removal of outgrown lashes. Consultation required before service. If you are ineligible, a refund will be provided. Not valid with other offers or promotions. May be repurchased every 30 days. Limit 3 per person.
