Option 1 : Immigration Physical Exam with Form I-693 Paperwork Two Visits

Prime Health Medical Center offers an immigration physical exam package for patients who need Form I-693 medical exam paperwork. This package includes the physical exam, medical-history review, vaccination-record review, evaluation, and Form I-693 paperwork preparation. The package includes two visits. The first visit is for the physical exam, evaluation, document review, and vaccination-record review. The second visit is for final paperwork review, signature, and pickup after all required information is complete. No fasting is required for this exam.

What Is Included

One immigration physical exam package with two visits. The package includes the physical exam, medical-history review, vaccination-record review, evaluation for Form I-693, paperwork preparation, final paperwork check, civil-surgeon signature, and pickup visit. Patients should bring all USCIS-required documents, valid photo ID or passport, A-number if available, and all available vaccination, blood work records to the first appointment.

The Groupon price does not include blood work, lab testing, TB/IGRA test, gonorrhea test, syphilis test, vaccine titers, vaccines, chest X-ray, treatment, translation, legal services, USCIS filing fees, or outside document fees. If any required lab work, vaccine titers, vaccines, chest X-ray, or treatment are needed, they will cost extra and will be discussed during the visit.

Patients should bring all available vaccination records to the evaluation visit, including records for OPV/IPV polio, MMR, varicella, hepatitis B, Tdap/Td, flu/influenza, and any other vaccines. If the patient does not have vaccine records, the patient can still come in for the appointment. Missing or incomplete vaccine records may require vaccine titers or vaccines at an additional cost.

Lab Result Timing

Some patients may need additional lab work or vaccine titers depending on USCIS medical exam requirements and their vaccination records. Lab results may take 5 to 14 business days depending on the test ordered. Final Form I-693 paperwork cannot be completed until all required documents, lab results, vaccine records, titers, vaccines, or other medical requirements are complete.

Important Patient Notice

The applicant must come in person with valid photo ID. No one else can complete the immigration physical exam, evaluation, paperwork review, or pickup process for the patient. The patient must bring all USCIS-required documents and all available vaccination records to the appointment.
