Get directions and the preliminary paperwork in advance. There are over half a dozen forms to complete. Be prepared for front office staff to not be on there best behavior. Was put on hold for over 10 minutes to make appointment. They never sent me the paperwork, and when I arrived they were not friendly, BUT Dr. Chance is easy to speak with, is a great listener and very knowledgeable so it is well worth it.
