Print off and fill out the new patient forms before you go to your appointment as they're pretty detailed and take a while to fill out, probably at least 30 minutes.

Very unprofessional service. Provider was running very late. Finally got into my appointment and she didn't listen at all. Gave me a ridiculous amount of supplements to take/ purchase. I won't be going back.
